The inspector’s account

Licensing Program Analyst (LPA) Alberto Lopez made an unannounced visit to conduct annual inspection. . LPA was met by Administrator Angela Nwaka and explained the purpose of the visit. This facility is licensed as an Adult Residential Facility and is serviced by San Gabriel/Pomona Regional Center. The facility is licensed to serve four (4) developmentally disabled clients ages 18 and above; of which all must be ambulatory. Currently there are two (2) clients in care.

1. Infection Control : Infection control practices and Personal Protective Equipment (PPEs) were observed. Facility is disinfecting throughout the day.

2. Physical Plant & Environment Safety: The facility is a single-story house located in a residential neighborhood. LPA toured the facilities physical plant, indoor and outdoor. Facility consists of three (3) bedrooms, including one shared room, two (2) bathrooms, living room, dining room, kitchen, laundry room, garage and an indoor/outdoor activity area. Passageways and one exit is free of obstruction. Other exit has tree branches from recently pruned tree blocking exits. Administrator stated it will be removed tomorrow. Client rooms are furnished with appropriate furniture for clients’ comfort. Bathrooms are operable and furnished with grab bars and nonskid surfaces. Common areas are observed for the ability to safely serve the needs of the clients. A shaded area with chairs is provided in the back yard. Kitchen appliances appeared clean and were operating at the time of the visit. One window screen needs repair and shared room needs holes repaired.

3. Operational Requirement: The facility is licensed for four (4) ambulatory clients and currently all two (2) clients are ambulatory. Clients can attend the community events/activities if there's an opportunity and chance.
