The inspector’s account

Licensing Program Analyst (LPA) Vaid conducted an unannounced annual inspection visit. LPA was allowed entry by Renee Atilano, Direct Staff Person and met with Jollie Rey Ramos, supporting administrator, and discussed the purpose of today's visit. Facility is licensed to serve four (4) non-ambulatory Developmentally Disabled clients who are age 18-59. Facility is approved for delayed egress and secure perimeters. Three(3) clients residing at this home receive case management services provided by San Gabriel Pomona Regional Center. Annual fees were current. Administrator Lee Strollo arrived shortly after, Administrator Lee Strollo certificate is current and expires on 06/05/2026.

For today's visit, CARE tool was used, staff/clients were interviewed, physical plant was conducted, and medications/food supply/staff files/client's files were reviewed.

The facility is a single-family home located in a residential neighborhood and consists of three (3) client bedrooms, one (1) full bathroom, one half (1/2) bathroom, living room, kitchen, den, dining area and laundry area. All rooms are in compliance, with comfortable furnishings. Bathrooms were clean and operable. Hot water temperature measured between 105-120 degrees Fahrenheit. Adequate linen and personal hygiene supply were in stock. Dual Smoke/Carbon monoxide detectors were tested and operable. The last Fire/ Emergency Drill was conducted on 10/10/25 and 10/24/25 for the NOC shift.

Delayed egress and audio device are working. Back yard activity area had shaded area with chairs and free of debris/ hazard. Sufficient supply of perishable and non-perishable foods was observed. Medications were centrally stored, locked and the records are current. Hazardous items are locked and inaccessible to clients. Fire extinguishers were fully charged, dated 11/05/25, and located in the kitchen and laundry closet. Comfortable temperature of 73 degrees Fahrenheit for clients was maintained.

No deficiencies cited per California Code of Regulations, Title 22, Division 6.

Exit conference is conducted. LIC 809 is discussed and provided to facility administrator.
