Facility Evaluation Report
Licensing Program Analyst (LPA) Vaid conducted an unannounced annual inspection visit. LPA met with Staff#1 Ionie Taibo-Direct Staff Person, who allowed entry and contacted the Administrator. S1 assisted with today's visit. LPA spoke with Administrator, Kenneth Njoku, over the phone, and arrived shortly after. Today's visit was discussed. The facility is licensed to serve six (6) developmentally disabled clients, ambulatory only, ages 18-59. Annual fees are current. Three clients are present, other three clients have gone to day programs.
Today's visit applied CARE tool, conducted physical plant, reviewed staff's files/clients' files/medication, interviewed staff/clients, and checked food supply.
The facility is a single-story home, consists of four (4) bedrooms, two (2) bathrooms, kitchen with a dining area, living room, laundry room and office in the garage. Facility maintains the required two (2) days perishable and seven (7) days non- perishable. Clients’ bedrooms are complying with furnishings, bed, lamp, chair, table. Adequate linen and personal hygiene supply were observed. Smoke detectors and carbon monoxide detectors were operable. Hot water temperature was measured between 105–120 degree F. Medications were properly logged and current. Fire extinguisher is fully charged. The front yard is well maintained. Fire drill was conducted on 5/01/25. Administrator Kenneth Njoku certificate expires 02/13/2026. Administrator Bolaji Akindmulero, certificated expires 2/25/2026.
No deficiencies were observed. An exit interview was conducted. Report was provided to Administrator(s) Kenneth Njoku and Bolaji Akindmulero.
