Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted a case management visit to inspect the facility for a capacity decrease from six (6) ambulatory to four (4) ambulatory clients. The purpose of the visit was explained to Administrator Tamaru Francis.
The facility is currently licensed serve six (6) ambulatory, developmentally disabled clients 18-59 years old. All clients in the facility receive services from San Gabriel/Pomona Regional Center.
The fire safety inspection for a capacity decrease was granted by Marc Bauer of County of Los Angeles Fire Department on 6/9/26.
Room #1 and #2 are private and room #3 will be shared room by two (2) clients.
Exit interview was conducted. A copy of the report was provided to Administrator Tamaru. Francis.
