Facility Evaluation Report
Licensing Program Analyst (LPA) Nune Margaryan conducted an unannounced annual visit using the Care Tool. LPA met with Administrator Sandy Empson who assisted with the visit. LPA explained the reason for the visit.
The physical plant was inspected along with medications, food supply, and resident and staff records. The facility is licensed to serve developmentally disable clients between the ages 18 to 59 and vendorized by San Gabriel/Pomona Regional Center. There are three currently (3) ambulatory clients reside in the home. At the time of visit clients were at the Day Program.
The facility is a single story house and located in a residential neighborhood area. The facility includes dining area, kitchen, living room, den, two (2) client's bedrooms, two (2) family/staff bedroom, one (1) client bathroom, one (1) staff bathroom (related to the staff room), laundry room and detached garage. The front and backyard are well maintained. The facility has a hot tub in the backyard, but it's covered up by the cover and it's not always using it. Passageways and exits are free of obstruction. There is a shaded seating area for the clients located in the backyard. Laundry was observed in the back of the kitchen, between facility and garage. LPA observed laundry detergent locked in the cabinet and not accessible to clients. There is a fireplace located in the living room area which is covered by the metal screen. Facility has an operable smoke / carbon monoxide detector, Client bedrooms were checked. All bedrooms have required furniture and beddings and sufficient lighting and closet space. Extra blankets, linens were observed in the closet located in the hallway. Client's bathroom was inspected. The client's bathroom is clean, sanitary and in a good working condition.
