The inspector’s account

Licensing Program Analyst (LPA) Nune Margaryan conducted an annual required visit. LPA met with staff Joel Mugo. The purpose of the visit was explained. Administrator Bundi Nyaga arrived shortly after and assisted with the visit. The facility is licensed to serve developmentally disable clients between the ages 18 to 59. Approved for four (4) ambulatory client. There are currently 4 clients residing at the home and receive services from San Gabriel / Pomona regional Center.

The physical plant was inspected along with medications, food supply, and client and staff records. LPA and Administrator toured the home and inspected (4) client bedrooms, (2) bathrooms, kitchen, dining room, living room, laundry area. Closed for extra beddings observed in the hallway. There is a shaded seating area for the clients located in the backyard. There are no pools or large bodies of water. LPA observed broken chair, dresser, bed frames and old mattress in the backyard. Client bedrooms were checked. Each bedroom is equipped with the proper furnishings. Bedrooms also have sufficient closet space. LPA observed in one of clients room curtain hanger was loose and metal wire used to secure it to the wall. The bathrooms were toured. Bathrooms are clean and have the required hygiene items. The water temperature was tested in the bathrooms and measured at 112.5F - 114.2F, which is within the required 105 - 120F degrees. LPA observed in clients bathroom the metal handle on the glass door of the shower was broken. The kitchen was inspected. There is sufficient perishable and non-perishable food. All the appliances are clean and working properly. Sharps are locked in a kitchen under the sink and are inaccessible to clients, however LPA observed screws, screw drivers, various metal scraps, batteries, light bulb unlocked in the kitchen drawer and accessible to the client.
