Licensing Program Analyst (LPA) Blanca Gonzalez generated this Case Management- Deficiencies report regarding observations made on 04/07/2026.

LPA placed a laptop on a table utilized by clients in the main room and the tabletop was loose and tilted when leaned on. LPA proceeded to inspect tables throughout the facility and observed 2 out 2 tables in the vocational room and 2 out of 3 tables in the main room to have tabletops that are loose and unstable. The loose tabletops pose a potential safety risk to clients in care due to lack of stability. Staff stated loose screws have been tightened before but tabletops are still unstable.

Deficiency cited under California Code of Regulations Title 22. Refer to LIC 809D.
