Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by staff and the purpose of the visit was explained. Program Director Laura Bradshaw and Assistant Program Director Evelyn Serrano arrived shortly after to assist.
The day program is licensed to serve a maximum of 30 non-ambulatory clients, ages 18 through 59, 13 wheelchairs permitted only. Facility is operating within the scope of their license.
The facility includes a main room, storage room, relaxation room, office, changing room, kitchen, two (2) client bathrooms, art room, vocational room and a front outside patio.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
The facility is clean, safe, free of obstructions and in good repair. Kitchen was observed clean and appliances were working properly. Food is not prepared at this program; however, clients are able to bring their lunch. The kitchen has a refrigerator in which clients can store food they bring from home.
