Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Case Management – Other visit to review staff records. LPA met with Laura Ovalle, Assistant Program Administrator, and the reason for the visit was explained.

During today’s visit, LPA obtained a copy of staff roster and reviewed facility files for Staff 1 – Staff 4 (S1-S).

Records review determined that S1 meets the requirements to provide the day program with administrative duties according to the following regulation:

CCR Section 82064(h)(2) Administrator - Qualifications and Duties (h): If an administrator is responsible for two or more adult day programs, there shall be at each site an employee who is responsible for the day-to-day operation of the program and who meets the following qualifications: (2) A minimum of one year of experience in a supervisory or management position in the human services delivery system.

No deficiencies noted during today’s visit. Exit interview was conducted with Laura Ovalle, and a copy of the report was provided.
