Facility Evaluation Report

At 10:00am Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced Annual/Required visit. LPA met with Shawn Mokricky, Operations Manager, and explained the reason for the visit.

On November 13 th , 2025, LPA received an email from the facility’s Regional Clinical Director (RCD) regarding room number 5 being flooded due to strong rains. LPA was also informed that the last client was going to be discharged from this facility as of November 13 th , 2025, and no additional clients will be admitted until the renovation is complete.

Upon arrival LPA observed that the facility is vacant. LPA was also informed the house required to have a window covering and chest of drawers replacement in all rooms, and repairing cracks in walls and repainting. No major renovation was observed during today’s visit. LPA was informed that the prospective reopening date will be December 1 st , 2025.

Operation Manager was informed to notify Community Care Licensing Division (CCLD) once the first client is admitted.

No citations were issued during this visit. Exit interview conducted. Copy of report signed and delivered.
