Facility Evaluation Report

On May 07, 2025 Licensing Program Analyst (LPA) Yolanda Rosser conducted an unannounced annual required visit using the Care Inspection Tool. LPA Rosser met with Chimenem Cyril-Eziwhou, and explained the purpose of today’s visit. The facility is licensed to serve 5 clients ages 18 to 59 years old, (5) five Ambulatory clients, of which (1) one non-ambulatory.

There are currently four (4) clients in placement. The facility consists of 4 bedrooms, 2.5 bathrooms, living and dining rooms, office, kitchen, laundry area contains washer & dryer located off the kitchen, front and back yards, detached garage.

LPA Rosser and Administrator, Cyril-Eziwhou toured the physical plant. There are no bodies of water or firearms on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured 110- 115.5 F. All fire extinguishers were operable. Last fire drill was on 01/14/25.
