Facility Evaluation Report
On 07/02/26 Licensing Program Analyst (LPA) Villegas conducted a case management visit in response to a call The Department received reporting clients were left without supervision. LPA met with caregiver Yolanda Baltazar as the purpose of the visit was explained.
During case management visit LPA Villegas conducted a Health & Safety visit which consisted of the following: tour of the physical plant and observation, LPA also requested a copy of staff roster (LIC 500) which per caregiver there was no staff roster available for review. LPA observed there were (4) clients in care with (1) caregiver present. LPA did not observe any immediate Health & Safety issues were observed.
