Facility Evaluation Report

On 11/17/2021 Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced Annual required visit with a primary focus on infection control measures. LPA Calderon was met by Administrator Roneshia Jones and the purpose of today’s visit was explained. The facility is licensed to serve 5 residents 18 to 59 years or older.

There are currently 5 residents in care. There are 3 ambulatory clients and 2 non-ambulatory clients in care. The facility is a one-story structure with 3 bedrooms and 1 bathroom, living room, kitchen, dining room, patio/back yard.
