The inspector’s account

On 05/11/2026, at 8:30 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. Upon arrival LPA met with Tomoya Henderson Support staff, and she was informed of the purpose of today’s visit.

The facility is licensed to care for adults ages 18 through 59,they are approved for four (4) ambulatory clients only. Currently, there are (4) clients in the home. The clients are South Central Los Angeles Regional Center (SCLARC) clients. There are no clients with restricted health care conditions, and none are utilizing postural supports or other protective devices.

The facility is a single story home and consists of 4 bedrooms, 2 bathrooms, one (1) office, living room, kitchen, dining room, a laundry area, garage with an additional refrigerator, deep freezer and extra cleaning products and a backyard patio area with a shed with outdoor supplies and two (2) outdoor shaded area

The facility annual fees are current. The liability insurance is with Philadelphia Insurance Co. and policy # PHPK2630089-012 effective 12/06/2025- 12/06/2026 with each occurrence at $1,000,000 and general aggregate $3,000,000.

At 8:50 AM, the department conducted a review of four (4) clients records which consisted of admission agreements, needs and service plan and IPP which appeared to be up to date. The department also reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings, which appeared to be up to date.
