The inspector’s account

On 05/20/2026 at 1:41pm, the Department made an unannounced visit to conduct the annual required visit using the CARE Inspection Tool. At 2:10pm, the Department met with Administrator Morigan S. Coffee and the purpose of the visit was explained. The facility is licensed to serve 4 ambulatory clients ages 18 - 59. Current census is 3. The clients are linked to the South Central Los Angeles Regional Center. Facility fees are paid in full.The facility has a current administrator certificate for Cherry Martin #7009549735 valid 10/17/2024 - 10/16/2026 and Morigan Coffee #7022540735 valid 07/10/2024 - 07/09/2026

The facility is a single-story structure located in a residential neighborhood and consists of the following: (3) bedrooms, (2) bathrooms of which (1) is private, living room, kitchen, dining area, garage, and back yard.

Between the hours of 2:15pm - 3:15pm, the Department conducted a records review of (3) client records, (4) staff records, (2) Client Medication Administration Records, (3) clients Personal & Incidental Records, and the facility disaster plan. All client & staff records were complete. The facility disaster plan was current (last conducted on 05/01/2026) and in compliance with Title 22 at the time of visit. The Department did not observed any discrepancies at the time of visit.

Between the hours of 3:15pm - 3:25pm, the Department and toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed , plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105°F -120°F (Kitchen 114.2°F, Bathroom #1 114.3 °F , & Bathroom #2 114.1 °F).
