Facility Evaluation Report
Licensing Program Analysts (LPAs) Martessa Brown and Mario Leon conducted an unannounced Annual required visit with a primary focus on infection control measures. LPA was met by Demarest Glenn and the purpose of today’s visit was explained. The facility is licensed to serve 6 developmentally disabled clients (age 18-59). All clients are Ambulatory.
There were no clients present due to being at the adult day program . The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms and 3 bathrooms, family/dining room, kitchen, living room, indoor and outdoor activity with shaded area, Laundry room area located in the washroom area.
LPAs and staff toured the physical plant. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked. All beds and bedding were in good condition and adequate lighting provided, storage for client personal belongings was observed. Walls and floors were in fair condition. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The hot water temperatures measured in 3 bathrooms at 113.8, 109. and 116.5 degrees Fahrenheit. A comfortable temperature is maintained in the facility. LPA observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to clients. The kitchen was inspected and there is enough perishable and non-perishable food available which is stored properly. Fire extinguisher was charged, smoke detectors were operational. There was no carbon monoxide in the facility, will purchase today.
