The inspector’s account

On 06/05/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual inspection visit , LPA met with Administrator Karen Carpenter as the purpose of today’s visit was explained. The facility is licensed to serve 4 Developmentally Disabled Clients (age 18-59). The clients are South Central Los Angeles Regional Center clients. (1) client with restricted health condition care plan. The facilities’ annual fees are current, surety bond (64467469) is active, liability insurance active (C1GP506348).

The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms, 3 bathrooms, dining room, kitchen, living room, outdoor shaded area, laundry room, and an attached garage. All client rooms were checked, beds and bedding were in good condition, adequate lighting provided, adequate storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations. Toilets and water faucets worked properly. The shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature in the kitchen measured 105-120F. Facility observed to be clean and in good repair. Storage areas for personal hygiene observed, toxins and sharps were inaccessible to clients. The kitchen was inspected and there is enough perishable and non-perishable food available. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked . The (2) fire extinguishers were charged, smoke detectors and carbon monoxide detectors were operable. The last emergency drill was conducted on 02/02/2025. There are no bodies of water or firearms on the premises.

LPA conducted a records review of (4) client records, (3) staff records, (4) P&I records, and (4) Medication Administration Records (MAR). No discrepancies at the time of visit.

Exit interview conducted with Administrator Karen Carpenter, and a copy of this report was provided.
