Complaint Investigation Report
The investigation revealed the following:
Allegation: Staff does not ensure sharp knives are made inaccessible to clients.
It is alleged that staff did not ensure sharp knives were made inaccessible to clients, resulting in a client obtaining a knife from the kitchen and chasing a neighbor with it.
On 04/13/2026 the Department conducted an interview with the Chief Executive Officer (CEO), Christopher Aaron (S1). S1 was asked if sharp objects/knives were kept locked away and inaccessible to clients in the facility, and S1 responded yes. The Department also asked S1 where C1 obtained the knife, and he stated that C1 told him he obtained the knife from a friend in the community.
On 09/03/2025 the Department interviewed Staff #1–#3 (S1–S3). Out of those interviewed, 3 out of 3 staff denied the allegation. On 09/03/2025 and 9/30/2025 the Department conducted interviews with Client #1-Client#2(C1 and C2). Out of those interviewed, 2 out of 2 clients denied the allegation.
On 09/02/2025 the Department conducted an interview with Witness #1 (W1). W1 said he was standing in front of the gate on the sidewalk right in front of the property when C1 came outside the facility with a knife. W1 also said the staff were shouting at C1 and telling him to come back to the house. W1 further stated that C1 went back into the house but came out again with a broomstick and then was called back in again and came out with a mop handle.
The Department conducted a tour with the Chief Executive Officer (CEO), S1, and inspected the kitchen for unlocked knives. The Department observed that the area where sharps were kept was locked and inaccessible to clients.
Based on the information gathered, interviews conducted, and records reviewed, the Department found no evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur; therefore, the allegation is Unsubstantiated.
Allegation: Staff does not prevent clients from engaging in physical altercations with other adults.
It is being alleged that staff did not prevent clients from engaging in physical altercations with other adults, as a client reportedly assaulted a neighbor’s wife and later chased the reporting party multiple times with various objects.
