Facility Evaluation Report
On 05/21/2026 at 1:33pm, the Department conducted an unannounced annual inspection visit using the CARE Inspection Tool. The Department met with the Vice President, Christopher Aaron the purpose of today’s visit was explained. The facility is licensed to operate for (6) ambulatory, (0) non-ambulatory developmentally disabled or Mentally Ill adults ages 18 through 59. Currently, the home has (1) client. The client is a South Central Los Angeles Regional Center client. The client does not have Restricted Health Care Conditions and does not utilize postural support or protective devices.
The facility is a one-story home located in a residential neighborhood. The property consists of the following: 5 client bedrooms, 2 common bathrooms, living room, kitchen, dining room, unattached garage, washer and dryer and an outdoor shaded area.
Between the hours of 2:30pm- 3:00pm, the Department conducted a records review of (1) client records (3) staff records (1) clients Personal & Incidental Records and reviewed the facility disaster plan. The facility disaster plan was current and in compliance with Title 22 at the time of visit. Also reviewed (1) Client Medication Administration Records.
Between the hours of 2:05pm - 2:25pm, the Department & Walter Banigo (DSP) toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed , plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to client. The water temperature properly measured between 105°F -120°F (Kitchen 116.1°F, Bathroom #1 112.2 °F , and Bathroom #2 119.6 °F).
