Facility Evaluation Report
On 6/10/2026, at 8:10 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. Upon arrival LPA met with Ashlee Johnson- Support Staff , and she was informed of the purpose of today’s visit.
The facility is licensed to operate and care for adults ages 18 through 59, approved for six (6) ambulatory residents only. Currently, the home has zero (0) clients. The facility is a three (3) bedroom, one (1) bathroom, single story story house.
The facility’s annual fees are not current. The PIN number and the total amount due were provided during the visit.The licensee Sophesias and Ashlee Johnson was also informed on 6/10/2026 that the annual fees must be paid by 6/12/2026.
At 9:10 AM, LPA Allen and Ashlee Johnson- Support Staff toured the physical plant. The bedrooms have the required beds and bedding supplies which appeared to be in good condition, adequate lighting is provided, and there is storage space for client’s personal belongings. there was additional bed linens, comforters, and bath towels were adequately stocked at the time of visit.
The bathroom appeared to be clean and operational. The water temperature ranged from 105 -120 F° and the facility temperature ranged from 72-75 degrees F.
LPA Allen observed that facilities kitchen to have a 5-day supply of perishables and a 7-day supply of non-perishables food items for the number of clients in care which were stored and maintained properly.
