Facility Evaluation Report
On 06/12/2025 Licensing Program Analyst (LPA) Jose Anguiano conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Edward Goodson and explained the purpose of today’s visit. The facility is licensed to ambulatory only, adults ages 18-59 years with a capacity of 16 residents and a current census of 16 client in care.
The facility consists of the following: a two-story structure located in a residential neighborhood with 10 bedrooms, 3 bathrooms, 1 kitchen with 2 dining areas, laundry is located outside, office areas located in the front, and outside area with shaded seating.
LPA toured the physical plant. There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident’s personal belongings was observed. Bed linens, comforters, and bath towels were stocked during the visit. Bathrooms were operational with hot water temperature measured at 115 degrees F.
