Facility Evaluation Report

On 7/24/2024 at around 11:00 AM Licensing Program Analyst (LPA), Leandro conducted an unannounced Case Management visit to follow up on deficiencies and plans of corrections. LPA met with the Administrator, Edward Goodson and explained the purpose of the visit.

Licensee did not clear 5 citations and received civil penalty assessment due to failure to correct.

Deficiencies cited under California Code of Regulations, Title 22, Division 6, Chapter 8.

An exit interview was conducted, and a copy of the Facility Evaluation Report along with Appeal Rights were provided to the Administrator.
