The inspector’s account

On 10/9/25, at 9:20am, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced visit to Congregate Connect-Arbutus. LPA was met by Arely Salazar, DSP, and Jessica Maloy, Administrator, and the purpose of today’s visit was explained. The facility is licensed to serve four (4) developmentally disabled clients (age 18-59) of which three (3) may be non-ambulatory. Currently, the home has (4) ambulatory clients. The clients are South Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none utilize postural supports or protective devices. The facility’s annual fees were paid on 10/9/2025 for $681.00 during the annual inspection.

The facility is a single-story building located in a residential neighborhood. It consists of the following: 4 bedrooms, 2 bathrooms, dining room, kitchen, living room, indoor and outdoor activity area, laundry room, patio area with chairs, and an attached garage.

LPA conducted a review of (4) client records, (11) staff records, (4) clients Personal & Incidental records and reviewed the facility disaster plan. LPA observed that clients had the following current reports in their file: Physician’s report, Admission Agreements, Behavioral Assessments, Quarterly Reports, and Regional Center Individual Program Plan (IPP). Staff records were complete with required documents and training. Clients P & I records were accurate. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) client medication administration records and m edication and did not observe any discrepancies at the time of visit.
