Facility Evaluation Report
On 07/16/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Lamont David floor supervisor as the purpose of today’s visit was explained. The facility is licensed to serve 32 ambulatory adults ages 18– 59 years. Clients are lined to SCLARC Facility fees are current.
The facility consists of the following: The facility is a one story building located on a main street and consists of the following: 1 receptionist room, 1 activity room, 1 conference room, kitchen area, 2 offices, 2 bathrooms, and a backyard patio area with shaded seating. Walkways around the facility were clear of hazards. There are no security bars, bodies of water, nor weapons on the premises. The Facility walls and floors were in good condition, storage space for clients to utilize, adequate lighting and sufficient furniture observed throughout the facility. Chemicals and sharps were properly locked, multiple first aid kits was fully stocked, restrooms were clean and within Title 22 regulations. A Land line and internet service were observed, smoke alarms and carbon monoxide are hardwired and operational. Clients bring their own meals and snacks
LPA conducted a review of (4) consumer records, and (4) staff records, no discrepancies observed. The day proram does not handle any client cash nor medications. The last fire drill was conducted on 06/12/26, LPA observed (2) fire extinguishers were observed throughout the facility.
Exit interview conducted and a copy of this report was provided.
