Facility Evaluation Report

On 05/30/2025, Licensing Program Analyst (LPA) Jose Anguiano Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Director Yolanda Green-Williams and explained the purpose of today’s visit. The facility is licensed to serve 32 ambulatory adults ages 18– 59 years. A total of 28 ambulatory clients are currently enrolled in this facility.

The facility consists of the following: The facility is a one-story building located on a main street. The facility consists of 1 receptionist room, 1 activity room, 1 conference room, kitchen area, 2 offices, 2 bathrooms, and a backyard patio area with shaded seating.

LPA toured the physical plant. There were no bodies of water on the premises. Hot water temperature measured at 110 degrees F. A comfortable temperature was maintained in the facility at 66 degrees F.
