Facility Evaluation Report
On 09/19/25, at 1:20pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced visit to Amazing Efforts VI LLC. LPA met with Christopher Brown, Direct Support Professional (DSP), and the purpose of today’s visit was explained. The facility is licensed to serve (4) ambulatory, of which (2) may be non-ambulatory (Developmentally Disabled or Mentally Ill) adults ages 18 through 59. Currently, the home has (4) ambulatory clients and (0) non-ambulatory clients. The clients are South Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none utilizes postural supports or protective devices. The facilities’ annual fees are current.
The facility is a single-story home located in a residential neighborhood. It consists of the following: 4 bedrooms, 3 client bathrooms, (1) staff bathroom, family room/office, lounge, living room, kitchen, dining room, shaded area, laundry room area, indoor and outdoor activity area, music room, vegetable garden, horse stables converted into storage areas, and a detached garage (game room).
LPA conducted a review of (4) client records, (12) staff records, (2) clients Personal & Incidental records and reviewed the facility disaster plan. LPA observed that clients had the following current reports in their file: Physician’s report, Admission Agreements, Behavioral Assessments, Quarterly Reports, and Regional Center Individual Program Plan (IPP). Staff records were complete with required documents and training. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) client medication administration records, and medication, and did not observe any discrepancies at the time of visit.
