Facility Evaluation Report

On 12/01/2022 at 1:32pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual inspection visit with a primary focus on infection control measures. LPA was met by Vanessa Kassel, staff and Ivette Gomez, administrator; and the purpose of today’s visit was explained. The facilities annual fees are current.

As part of the inspection, my primary focus was on infection control. LPA observed the facility’s infection control practices: LPA observed a sanitizing station at the facility entrance. LPA’s temperature was taken upon entry. PPE supplies are readily available to staff and residents, and additional supplies are stored. Sufficient paper, cleaning, and disinfecting supplies were observed. The facility has the mandated COVID infection control posters.

There are currently (4) four Regional Center consumers in placement. All (4) clients are ambulatory. The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms, 4 bathrooms, family room/office, lounge, living room, kitchen, dining room, shaded area, laundry room area, indoor and outdoor activity area, music room, vegetable garden, horse stables converted into storage areas, and a detached garage (game room).
