Facility Evaluation Report
On 06/20/25, at 09:00am, Licensing Program Analyst (LPA) Villegas conducted an unannounced visit to the facility. The purpose of today’s visit was to conduct the required annual inspection, using the new Care Tool. LPA was met by Licensee Joseph Avery, as the purpose of today’s visit was explained. The facility is licensed to serve 15 Developmentally Disabled Clients (age 18-59) ambulatory only. There is currently (0) clients at the Adult Day Program. The facilities’ annual fees that were due on 06/18/2025 and has a balance due of $90.50, Licensee provided with annual fees info and Pin.
The program is a single story building located on the main street in a commercial area. The program has iron bars on the windows and contains an open lobby near the front entrance, (4) rooms that consist of a dining room, a computer lab, a mock convenience store room, and a quiet/calming room. There is 1 bathroom with 1 sink, a commode and a urinal. The program walls and floors were in good condition, storage space for consumers to utilize, adequate lighting and sufficient furniture observed throughout the facility. Chemicals and sharps were properly locked, first aid kit was fully stocked, restrooms were clean and within Title 22 regulations. Smoke alarms and carbon monoxide are operational. Program currently has (0) clients, however program does not provide lunch as clients will bring their own meals. Program does not assist with medications, LPA observed (3) fire extinguishers were observed throughout the program. There are no bodies nor obstructions on the premises, walkways throughout the day program and all exits were clear of hazards and debris.
Exit interview conducted, appeal rights obtained, and a copy of this report was provided.
