Facility Evaluation Report
On 10/09/2025, Licensing Program Analyst (LPA) Jose Anguiano conducted an announced annual required inspection and toured the facility with Administrator Leroy Allen. The facility has no clients in care at the time of the visit.
The facility is licensed to serve adults ages 18 to 59 and has an approved fire clearance for six (6) ambulatory clients. The facility is a single-story home consisting of three (3) bedrooms and one (1) bathroom. During the inspection, LPA observed the living room, dining area, kitchen, and garage. All inspected areas were equipped with the required furnishings, adequate lighting, and sufficient storage space. The front yard was clean and free of hazards. The backyard patio area was accessible; however, LPA observed wires and a metal antenna on the ground. LPA also toured the garage, which was locked and used for storage. Smoke and carbon monoxide detectors were tested and found to be operational. Water temperature in the kitchen measured 114°F. A locked storage area for medications and sharp objects was observed. Fire extinguishers were observed in the facility. Technical assistance was provided to ensure the garage is cleaned and organized, and to remove the wires and metal antenna from the backyard to eliminate potential hazards.
No deficiencies are being cited based on observations and record review, in accordance with Title 22 of the California Code of Regulations. Advisory Notes were issued on LIC-9102. A plan of correction was discussed and developed with the Administrator. An exit interview was conducted, and a copy of this report was provided to Administrator.
