Licensing Program Analyst (LPA) Jose Villalobos conducted a case management visit. LPA met with Staff Daisy Fitter and the purpose of the visit was discussed.
The purpose of the case management is to deliver and receive signature for an amended 9099-D page for complaint control # 28-AS-20201124163559 . No further investigation was conducted by LPA Villalobos and no other deficiencies are cited on today visit.
