The inspector’s account

Licensing Program Analyst (LPA) Elizabeth Irra conducted the required annual inspection. LPA met with Janin Balmaceda (Assistant Director) and discussed the purpose of today’s visit.

This program operates Monday through Friday 8AM to 2PM. This program is licensed for (25) ambulatory clients and (5) non-ambulatory clients. There are a total of (18) clients participating in this program. At the time of visit, (6) clients were present for in-person services. There are (17) clients that receive case management services provided by San Gabriel Pomona Regional Center and (1) client that receives case management services provided by Inland Regional Center.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Facility has an Infection Control Plan in place.

Physical Plant & Environment Safety: Carbon monoxide observed. Fire extinguishers observed (1) located in the front near the front restroom and (1) located in the back near the storage room (last serviced on 03/29/24). Hot water temperature in the front restroom measured at 117.3* and back restroom measured at 116.9*. Filtered drinking water is available for clients.

Operational Requirements: Staff are adhering to operational requirements.

Staffing : There is sufficient staffing at the facility. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility.
