Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit-Other on 05/23/2025, due to the pending closure of facility on 5/21/2025. LPA was greeted by Erika Montoya-Assistant Administrator and explained the purpose of the visit.
LPA conducted a physical plant inspection to verify the facility no longer houses residents. LPA Ramirez did not observe residents in the facility. All residents have been relocated. Per Assistant Administrator- Erika Montoya, no direct care staff will be working at the facility. Assistant Administrators Erika Montoya agreed to mail original facility license to LPA Ramirez by 05/30/2025.
No deficiencies were cited during this visit. Exit interview was conducted. A copy of this report was provided email.
