The inspector’s account

Licensing Program Analyst (LPA) Kimberly Ramirez and LPA Gabriela Castro conducted an unannounced Case Management Visit-Other on 05/20/2025, due to the pending closure of facility on 5/21/2025. LPA's were greeted by Administrator Ricardo Lara-Perez and LPA explained the purpose of the visit.

LPA's conducted a physical plant inspection and inspected food supply. Facility staff were observed providing care and supervision. Administrator Lara-Perez revealed Long Term Care Ombudsman (LTCO) has been assisting the facility with relocation of some residents. Administrator Lara-Perez revealed caregivers are on 12hr shifts.

No deficiencies were cited during this visit. Exit interview was conducted. A copy of this report was provided via email.
