Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Visit-Other on 04/15/2025, due to the pending closure of facility on 5/21/2025. LPA was greeted by Erika Montoya-Assistant Administrator and LPA explained the purpose of the visit.
LPA Ramirez conducted a physical plant inspection and inspected food supply. LPA Ramirez observed staff conducting seated exercises with residents in facility living room. Facility staff were observed providing care and supervision. Interview with Assistant Administrator Erika Montoya revealed facility staff are working 12 hours shifts to ensure resident care is not compromised. Montoya revealed Long Term Care Ombudsman (LTCO) has been assisting the facility with relocation of some residents.
No deficiencies were cited during this visit. Exit interview was conducted. A copy of this report was provided via email.
