The inspector’s account

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced required Annual Inspection visit on 07/23/2026. LPA was met by Administrator Leilani Libang and explained the purpose of the visit. This facility is licensed as an Adult Residential Facility for Persons with Special Healthcare Needs (ARFPSHN) and receives services from San Gabriel/Pomona Regional Center. The facility is licensed to serve five (5) developmentally disabled clients ages 18 and above; of which five (5) may be bedridden. During today’s visit, LPA Ramirez observed one (1) Respiratory Therapist, one (1) Licensed Vocational Nurse and three (3) Direct Support Staff.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Physical Plant and Environment safety: Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to clients, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected five (5) client rooms. All client bedrooms contained the required furniture, linens and lighting. One (1) client bedrooms is currently unoccupied. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees F. LPA Ramirez observe postings encouraging proper hand-washing etiquette in restrooms. LPA Ramirez observed grab bars near toilets and inside showers. LPA Ramirez observed non-slip coating in showers. Mechanical lift in bathroom# 2 was operable and tested during visit. LPA Ramirez observed a shower gurney in bathroom #2. See 809-C
