Facility Evaluation Report

Licensing Program Analyst (LPA) conducted an unannounced required annual inspection visit. LPA was greeted by House Manager Teri Austin and the purpose of the visit was explained. Administrator Kimberly Allen was called and notified of the visit.

The facility is licensed to serve four (4) adults age range 18-59, approved for one (1) ambulatory and three (3) non-ambulatory. Bedrooms # 3 and #4 okay for non-ambulatory. All clients in facility are receiving services from San Gabriel/Pomona Regional Center. Currently one (1) client over the age of 59.

The facility is a single-family home located in a residential area of Claremont. The home consists of a living room, dining area, kitchen, 3 client bedrooms, 2 bathrooms, an office, attached garage, front and back yards.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Living room and dining area were observed clean and free of obstruction with sufficient seating. Living room contained a fireplace that had a screened cover making it inaccessible to clients in care. Games and activities were observed stored in dining area.
