Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced case management visit. The visit is to deliver an amended report for complaint 28-AS-20260106103917 for the purpose of removing confidential information. Upon arrival, the LPA was greeted by Teri Austin, Facility Manager and the reason for the visit was explained.
There are no deficiencies observed during today’s visit. An exit interview was conducted, and a copy of this report was provided.
