Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced subsequent Case Management-Other visit on 07/24/2025, regarding special incident report received on 05/02/2025. On 05/08/2025, LPA Ramirez conducted a Case Management Visit- Incident to gather documents. During today’s visit, LPA Ramirez was greeted by Administrator Ifeoma Osonwa and explained the purpose of the visit.

On 05/02/2025, LPA Ramirez received Facility/Program Special Incident Report (SGPRC 641), reporting alleged sexual abuse of client#1 (C1) by staff#1 (S1). On 05/07/2025, C1’s placement agency was notified regarding alleged sexual abuse of C1 by S1. After review of documents collected on 05/08/2025, interviews of staff, residents, and C1’s family conducted by Community Care Licensing-Investigations Branch; revealed there is no evidence to support sexual abuse of C1 by S1. Review of Criminal Report completed by Claremont Police department revealed sexual abuse allegations were “unfounded”.
