Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management- Incident visit on 05/08/2025, regarding special incident report received on 05/02/2025 . LPA Ramirez was greeted by Direct Support Staff Jane Garcia and explained the purpose of the visit. Administrator Ifeoma Osonwa arrived shortly after to assist.
On 05/02/2025, LPA Ramirez received Facility/Program Special Incident Report (SGPRC 641), reporting alleged personal rights violation of client#1 (C1) by staff#1 (S1). On 05/08/2025, LPA Ramirez arrived to the facility and conducted a physical plant tour and did not observe any immediate health and safety concerns. LPA Ramirez obtained copies of C1's client file and S1's personnel record. Per Administrator Osonwa, S1 has not been back at the facility since 04/27/2025 and is not scheduled to return until further notice.
Based on documents gathered and reviewed, LPA Ramirez may return to gather additional documents or conduct interviews.
No deficiencies were cited during this visit. Exit Interview was conducted. A copy of this report was provided.
