The inspector’s account

Licensing Program Analyst (LPA’s) Gabriela Castro conducted an unannounced required annual visit using the Compliance and Regulatory Enforcement (CARE) Tool. LPA was greeted by Irene Aquino and explained the purpose of the visit. Administrator Ifeoma Osonwa arrived shortly thereafter.

Facility is licensed to serve six (6) developmentally disabled adults, ages 18-59. Two (2) can be non-ambulatory.

All six (6) clients residing at this facility receive case management services by San Gabriel Pomona Regional Center.

Personal Rights postings were observed in a common area. Residents had access to personal space, privacy, and adequate storage. No firearms/weapons were present.

Physical Plant The facility is a one-story residential home consisting of three (3) client bedrooms, a kitchen, dining room, living room, two (2) bathrooms, an attached garage with storage and laundry area, and a backyard patio. All three (3) bedrooms were observed to contain the required furnishings, including a bed with a clean mattress, linens, dresser, chair, and adequate lighting. Extra clean linens and towels were stored in hallway cabinets, and all furniture was observed to be in good condition. Both bathrooms were equipped with functioning sinks, toilets, and showers, and contained the required grab bars and non-skid mats. Hot water temperatures measured 107.2°F in Bathroom (1) and 108.5°F in Bathroom (2), which is within the required range of 105°F–120°F. Smoke and carbon monoxide detectors were tested and found to be operational, and a fully charged fire extinguisher was mounted in the kitchen area. Passageways and exits were observed to be clear and unobstructed, and no bodies of water were present on the premises. The backyard provided shaded seating for residents, and emergency food and water supplies were observed. All chemicals and cleaning supplies were secured and inaccessible to residents, stored in locked cabinets under the kitchen sink and in the laundry room. Sharp items were also secured in a locked kitchen drawer.
