The inspector’s account

Licensing Program Analyst (LPAs) Kimberly Ramirez and Gabby Castro conducted an unannounced Annual Required Visit on 07/22/2025. LPAs were met by Licensee Mona Delarosa and explained the purpose of the visit. The facility is licensed serve four (4) developmentally disabled clients 18-59 years old, of which four (4) may be non-ambulatory. The facility is approved for delayed egress. Surveillance video was observed on the outside perimeter of the facility. All clients in the facility receive services from San Gabriel/Pomona Regional Center.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control- Staff are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place.

Physical Plant & Environmental Safety- Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to clients. LPA Ramirez observed carbon monoxide detectors and smoke alarms in hallways. LPA Ramirez inspected four (4) client rooms. Three (3) out of four (4) the client bedrooms contained required furniture, linens and lighting. C1 has an exception granted for regulation 85088(c)(2)(3), however, LPA Ramirez observed one (1) nightstand dresser in C1’s bedroom. LPA Ramirez will issue Technical Advisory based on observation. Staff immediately removed the nightstand. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees.
