The inspector’s account

Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced Required Annual Inspection on the above date. LPA was greeted by Helen Piper, Administrator, and provided an explanation for the purpose of the visit.

The facility is a single-story residential home located on a main street and is licensed to serve six (6) developmentally disabled adult clients, of whom two (2) may be non-ambulatory. At the time of the inspection, the facility was unoccupied.

Cameras were observed in the common areas living room, dining room and hallway.

Front Yard: The front yard was observed to be well maintained and free of any hazards.

Kitchen: Kitchen appliances were observed to be in proper working order.

Dining area contained one table with six (6) chairs and another table with two chairs.

Thermostat in the common area registered at 70°F.

All rooms were observed to be furnished with appropriate bedding, linens, lighting, and required furnishings.

All rooms are shared and currently unoccupied.
