Facility Evaluation Report
At 11:30am, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. LPA met with the Staff, who granted access to the facility. The Administrator was contacted and LPA explained the reason for the visit.
A tour of the physical plant was initiated at 11:50am, and the following was observed:
The facility is still pending vendorization by North Los Angeles Regional Center and currently has no clients in care. The facility is approved for capacity of four (4) Ambulatory clients. Required postings were observed in the entry area. The facility maintains a comfortable temperature of 77°F. LPA observed all common areas are furnished with adequate furniture to accommodate a maximum capacity of four (4) clients. There is a functioning telephone on the premises.
The kitchen is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of four (4) clients. Sharp knives/objects are kept in the kitchen cabinet and observed to be locked and inaccessible to clients in care. The fire extinguisher is located near the kitchen and observed to be full. The laundry room is located by the kitchen. The washer/dryer appeared to be in good condition. Laundry supplies and chemicals are kept inaccessible to clients in care. Medication is kept in the hallway closet and observed to be locked. The first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual.
LPA observed 4 bedrooms designated for clients’ use. All bedrooms are furnished with beds and dressers and required bedding and linen. The bedrooms have sufficient closet space. There is one bedroom designated for live-in/awake staff. Continue on LIC809-C
