Facility Evaluation Report

In conjunction to the complaint 31-AS-20260526085332 LPA Mariana Agban conducted a Case Management- Deficiencies visit. During the course of the investigation, it was determined that Resident #1 (R1) eloped from the facility on 05/22/26. The investigation further revealed that the facility did not submit a Special Incident Report (SIR) to Community Care Licensing (CCL) as required. The Administrator acknowledged that the incident was not reported to CCL within the required time frame.During the visit, the Administrator provided the LPA with a copy of the completed Special Incident Report regarding the incident.
