Facility Evaluation Report

Licensing Program Analyst(LPA) Nicol Wesley conducted a required 1 year visit with inspection and met with Damieka Lasley. T his home is licensed to serve (2) non-Ambulatory and (1) Ambulatory clients between the ages of 18-59. The vendor is Harbor Regional Center. There were (3) clients in care during the time of this visit. The facility phone number is 562 474 8517.

The Compliance and Regulatory Enforcement (CARE) tools was used for the visit today. LPA Wesley toured the facility and observed the following: 3 bedrooms, 2 bathrooms, 1 living room w/ office area, Den with covered fireplace, laundry closet, kitchen, dining room and an attached garage. Hot water was tested and the temperature measured at 108.1 degrees F. Medications, medication records, and first aid kit with manual will be centrally stored in a locked office cabinets, toxins, detergents, and sharps, are stored in a locked office cabinets. Linen, towels, and PPE supplies, were observed in a closet in hallway. The smoke detectors/carbon monoxide detectors were tested and are operable. 1 fully charged fire extinguishers(1 in the kitchen area). LPA Wesley observed an adequate supply of perishable and non perishable food items, as well as an emergency supply of food and water located in the garage. There are no deficiencies cited.
