Facility Evaluation Report

On 6/30/2026, Licensing Program Analyst(s) (LPA) Jewel Baptiste conducted an unannounced annual visit at Semora ARF Inc., using the CARE tool. LPA Baptiste met with Administrator Julie Machado and explained the reason for the visit.

The facility is licensed to serve age range 18 through 59. Fire clearance was approved for three (3) ambulatory and one (1) non-ambulatory. The facility is a single-story home, located in a residential area. It consists of a downstairs 4-client bedroom, 2-client bathroom, 1 dining room, 1 living room with office space, kitchen, attached garage, a front yard and backyard.

LPA Baptiste conducted a tour of the facility with Administrator Machado and observed the following: Facility is clean and in good repair indoors and outdoors. Kitchen is clean, sharps were locked and inaccessible to clients. Fire extinguisher observed in kitchen, and garage charged and updated. The cleaning supplies were locked under kitchen sink and garage. Sufficient food supplies were observed, at least 2 days of perishables and 7 days of non-perishables. (4) client's bedrooms were observed with sufficient lighting, furniture, and bedding supplies. (2) client bathrooms were observed in working conditions with the water temperature tested between 113.5 -117.0, which is within the required 105-120 degrees F. The dining room and living room were observed with sufficient sitting area. Smoke/Carbon monoxide detectors were tested and in working condition. The garage contained storage supplies, PPE’s, washer and dryer, cleaning supplies, toiletries and extra food. Backyard was observed with a covered sitting area and a shed that contained holiday decorations.
