Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced Required 1 year visit at the facility and met with staff Tess Colobong, Arnold Villanino, Melda Ramirez to discuss the purpose for todays visit. Shortly afterwards MJ Nora and Normita Daclison arrived and joined the visit.

The Compliance and Regulatory Enforcement (CARE) tools was used for the visit today. LPA Wesley toured the facility and observed the following: 5 bedrooms, 2 bathrooms, living room, office/family room, kitchen, dining room and an attached garage. Hot water was tested and the temperature measured at 110-113 degrees F. Medications, medication records, and first aid kit with manual is centrally stored in the office cabinets. Toxins, detergents, and sharps, are stored in locked cabinets. Linen and towels, were observed in the hallway cabinet. The smoke detectors/carbon monoxide detectors were tested and are operable. 1 fully charged fire extinguishers(1 in the kitchen area). LPA Wesley observed a supply of perishable and non perishable food items. Last emergency disaster was completed on 03/01/2025.
