Facility Evaluation Report

On 4/23/26, Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced Annual/Required inspection in conjunction with a case management to Horst Home Care. Upon arrival LPA was greeted by Direct Support Professional (DSP) Kimberly Sauceda. The Administrator, Yetzira Anselmo arrived at 12: 50 p.m., and LPA explained the reason for the visit.

The incident is regarding a death of a client (C1). At the time of passing the client was at the day program and not in the home. During today’s visit LPA Baptiste reviewed files for C1 and collected Identification and Emergency Information, Physician’s report, Last Doctors visit, Medical Administration Record (MAR) for March, IPP, Incident report and Client Development and evaluation report. The Administrator stated C1 showed no signs of illness and family had visited C1 that morning and cut their hair. Per the Administrator the day program noticed C1 was sleepy and went to check C1’s oxygen level. While checking the oxygen level C1 lips turned white and they started to do CPR and contact 911. C1 was pronounced decease at the day program. It is suspected that C1’s death is related to cardiac arrest. The facility agreed to get the death certificate and send to licensing.
