Facility Evaluation Report

Licensing Program Analyst(LPA) Nicol Wesley conducted a required 1 year visit with inspection and met with Damieka Lasley. This home is licensed to serve (1) non-Ambulatory and (2) Ambulatory clients between the ages of 18-59. The vendor is Harbor Regional Center. There are (3) clients in care. The facility phone number is 562 584 4276.

The Compliance and Regulatory Enforcement (CARE) tools was used for the visit today. LPA Wesley toured the facility and observed the following: 3 bedrooms, 2 bathrooms, living room, kitchen, Office area, dining room, laundry room and an attached garage. Hot water was tested and the temperature measured at 112.5 degrees F. Medications, medication records, and first aid kit with manual will be centrally stored in a locked office cabinets, toxins, detergents, and sharps, are stored in locked in cabinets. The smoke detectors/carbon monoxide detectors were tested and are operable. There were (2) fully charged fire extinguishers located in the kitchen and laundry room. LPA Wesley observed an adequate supply of perishable and non perishable food items. The garage contained storage supplies including PPE, and water.

There were no deficiencies cited. Exit interview conducted and a copy of this report was given to the Administrator.
