Facility Evaluation Report

On 4/17/2026 Licensing Program Analyst (LPA) Jewel Baptiste conducted a case management visit in response to the death of Client #1 (C1), who passed away 4/8/26. LPA met with Judith De Los Reyes Direct Support Professional (DSP) and explained the purpose for today’s visit. Administrator Edith Parreno joined the visit at 11:30 am.

On 4/13/2026 the Department received a Death Report indicating that on 4/8/26 at approximately 11:30 pm C1 expired at UCI Health- Lakewood, C1 was sent to the hospital on 3/31/2026 due to having a seizure, unresponsive and was turning gray. C1’s family made the decision to place C1 on do not resuscitate. Per the Administrator C1 went into Cardiac Arrest and was pronounced decease on 4/8/2026.

During the case visit, LPA interviewed the Administrator and obtained copies of the following documents from C1's file: Face Sheet, Death Report, Physician's Report, Physicians Orders, Regional Center IPP, Needs and Service Plan, Medication List, last doctor’s visit. LPA also toured C1's bedroom. No concerns, obstructions, or anything out of the ordinary witnessed observed. LPA requested facility to obtain and provide Licensing with C1's Death Certificate upon receipt.

Facility contacted the family and stated they will get the death certificate from the funeral home and send to licensing.

An exit interview held, and a copy of this report was provided.
